Patient Representative Group

The patient group is a group of patients who meet on a 2-3 month basis to develop relationships between patients and the practice.

The objectives are to:

  1. Create and improve communication between patients and the practice
  2. Provide opportunity for patients to comment on the way facilities and services are planned and influence decisions on those services
  3. Provide constructive feedback, both positive and negative.
  4. Support the practice in promoting good health within the practice
  5. Collect patient opinions and experiences to help the practice evaluate its services
  6. Bring patients and the practice together

What is the Patient Representative Group (PRG)?

The PRG is a group of patients who are representative of the practice i.e. age, sex, ethnicity etc. these patients may not wish to attend the patient group meetings but we would still like to have the views of these patients. If you would like to be a part of the patient group but don’t have the time to attend the meetings, join our Patient Representative Group by completing the form on the “Contact Us” page of our website.  When we have your details we will contact you periodically to ask your opinions and get feedback on new and existing services within the practice.

The contact details of PRG members will not be shared, only anonymised comments/feedback will be passed to PPG or Practice members.

If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance
